The Assertive Community Team is a multi-disciplinary team that provides mental health interventions in the community. They work with smaller caseloads more intensively as a team to assist the counties most severely and persistently ill and vulnerable clients manage all their needs so they can remain living independently and safely in our community. They are a boots on the ground team that utilizes a trauma informed, humanistic, and wellness recovery model to work with their clients. The team meets the clients where ever they are whether it be their home, RTH, homeless shelter, or in the community to provide support. We focus on building strong therapeutic relationships, mental health symptom management, trauma, substance abuse, daily living needs, assisting with housing, managing physical health needs, and overall management of the social service benefits to ensure they continue to get the services they need to progress in their lives.
